New York State Freedom of Information Law Request

To submit an electronic request for existing records under the New York State Freedom of Information Law, please complete the form below or email DOCCSFOILUnit@doccs.ny.gov. Note that we will be unable to process incomplete or inaccurate entries.

Please be advised that within five business days this agency will respond to your request for records with a written acknowledgement of the receipt of such request and a statement of the approximate date, which shall be reasonable under the circumstances of the request, when such request will be granted or denied. If circumstances prevent disclosure to the person requesting the record or records within twenty business days from the date of the acknowledgement of the receipt of the request, this agency will state, in writing, both the reason for the inability to grant the request within twenty business days and a date certain within a reasonable period, depending on the circumstances, when the request will be granted in whole or in part. You will also be informed if the requested records are not maintained in electronic form, and that there will be a $.25 per page copying fee for any responsive documents.

Please be advised that all electronic Freedom of Information requests are determined to have been received on the first normal business day after such requests are submitted.
